This is the "Edit Architect" form. It is called from within the "Edit infobox" link on any page using the Arch template. The New Architect form inserts templates that should be included on every architect page, {{Biography}} and {{NewArchitectPageFormat}}, and calling that form while editing an architect's page inserted them again. So this form is now called from the Arch template, which is identical to the New Architect form, but without including those templates.
